I hereby grant to Ronald McDonald House Charities (“RMHC”), its local Chapters and programs, advertising and promotional agencies, and their agents (collectively, “RMHC”), the irrevocable and unrestricted right to use, publish, display, and distribute materials bearing my name, voice, likeness, or any other identifiable representation of myself.

These materials may appear in any form, style, color, or medium whatsoever, now known or hereafter developed, including but not limited to photographs, video, film, audio recordings, print, broadcast, and electronic or digital media.

I agree that all such materials, including any negatives, prints, recordings, or files, are and shall remain the sole and exclusive property of RMHC. I hereby assign any rights I may have in such materials to RMHC and release RMHC from any and all liability related to their use.

Release and Waiver of Liability

I hereby agree, on behalf of myself and my heirs, executors, administrators, and assigns, to voluntarily release, waive, discharge, and relinquish any and all actions, claims, or causes of action for bodily injury, personal injury, property damage, or wrongful death arising out of or related to my stay with RMHC and Sethi Properties.

This includes, but is not limited to, the use of RMHC facilities, services, and transportation, including golf cart or shuttle use.

I agree to hold harmless RMHC and Sethi Properties from and against any and all such claims, except for those arising from the willful misconduct of RMHC or Sethi Properties.

California Civil Code Section 1542 Waiver

I expressly waive all rights under California Civil Code Section 1542, which states:

“A general release does not extend to claims which the creditor does not know or suspect to exist in his/her favor at the time of executing the release, which if known would have materially affected his/her settlement with the debtor.”

Acknowledgment

I acknowledge that I have fully read and understand this agreement and that any questions regarding its meaning or effect have been answered to my satisfaction. I am signing this agreement voluntarily and certify that I am at least 18 years of age, unless this document is also signed by a parent or legal guardian.

Media Sharing Preference

From time to time, RMHC materials may be shared with RMHC Global to highlight their support of RMHC.

It is expected that families using the House will adhere to the guidelines. Violations should be addressed individually and repeated concerns or significant disregard of guidelines, as determined by RMHC House staff in its sole discretion, may result in losing access to the RMHC. All guests using the House must abide the guidelines, including but not limited to the following:

• Badges must be worn during your stay and returned upon your departure from the House.
• RMHC has the right to limit amount of guests staying or in the room.
• Upon approval, families staying over a period of three weeks are allowed (2)-24 hours leave during their stay. These 24-hour leaves cannot be in consecutive days.
• Smoking is not permitted, flammable materials are also not allowed anywhere in the RMHC, such as, candles, E cigarettes, ect...
• Alcoholic beverages or illegal substances (drugs) will not be tolerated anywhere in the premises including the parking lot.
• Share in the responsibility of keeping the RMHC House clean. We ask families to please clean up after themselves in the kitchen, bedrooms and common areas.
• Food and beverages should remain in kitchen/dining area only, not permitted in bedroom or living rooms.
• After visiting hours, no visitors are allowed in House or parking lot after 9:00pm.
• Supervise children at all times, no exceptions.
• Proper attire and footwear is required throughout the House.
• Persons who have or have been exposed to a contagious illness will not be allowed to stay in the House.
• No pets allowed in any areas of the House including the parking lot.

Registered families exceeding more than a 30- day stay are required to transfer to another RMHC room upon availability, for maintenance purposes. I have read, understand and agree to abide by the Guidelines of the Ronald McDonald House. I understand that violations or disregard of the guidelines, as determined by the RMHC staff in its sole discretion, may result in immediate loss of access to the House. We also understand that should a family w/greater needs be found, we may be asked to find alternative accommodations and in this event, we will be given a grace period of 24 hours to vacate my room.

Guest Agreement/Consent to Abide by Ronald McDonald House Sethi Properties Guidelines

Families using Hotels of the Heart Program are expected to adhere to the guidelines of Ronald McDonald House (RMHC) and Sethi Properties. Violations will result in loss of access to the Ronald McDonald House (RMHC) and its support services. All guests using the Hotels of the Heart Program must comply with the following guidelines, including but not limited to the following.

• Families must provide a valid credit or debit card to cover incidental charges or damages (no exception)
• All families must check in at Sethi Properties between 3:00pm -6:00pm (no exception)
• We are zero tolerance for domestic disputes and all actions will be reported to the authorities.
• RMHC and Sethi Properties have the right to limit room capacity to 4 people
• Smoking or flammable items, including items such as candles or e-cigarettes, are not allowed
• Alcohol or the use of illegal substances (drugs) will not be tolerated anywhere on the premises, including the parking lot.
• Share the responsibility of keeping the room in the best possible condition.
• Supervision of children at all times (we do not allow children to be left unsupervised in the room)
• Pets are not allowed in any area of the hotel unless a permit is granted for service animals

I have read, understand, and agree to abide by the Guidelines. I understand that violations or failure to comply with the guidelines, as determined by RMHC staff in their sole discretion, may result in immediate loss of access to the House.

Se espera que las familias que utilizan la Casa se adhieren a las guías. Violaciónes deben dirigirse de forma individual y preocupaciones repetidas o significativo desconocimiento de las guías, según lo determinado por el personal de RMHC Casa a su sola discreción, puede resultar en la pérdida de acceso a la Casa RMHC. Todos los huéspedes que utilicen la Casa deben acatar las guías, incluyendo pero no limitado a lo siguiente:

• Insignias son para ser usados durante su estancia y devueltas a su salida de la Casa.
• Una vez aprobado, las familias que permanecen durante un período de tres semanas o mas se les permite ( 2) permisos de ausencia de 24 horas durante su estancia. Estas ausencias de 24 horas no pueden estar en días consecutivos .
• Horas de visitantes son de 9:00am hasta 9:00pm, en todos lugares de la Casa incluyendo el estacionamiento.
• No se permite fumar, materiales inflamable, tabaco, veladoras, no se tolerará uso alcohólico/ o el uso de sustancias ilegales (drogas).
• Pedimos su participación en la responsabilidad de mantener la limpieza de la RMHC Casa. Les pedimos a las familias favor de limpiar ellos mismos en la cocina, dormitorios y zonas comunes.
• Comida y bebidas deben permanecer en la cocina / comedor solo, no son permitidas en el dormitorio o salas de estar.
• Superviso de los niños en todo momentos.
• Se requiere vestimenta adecuada y calzado en toda la casa.
• Las personas expuestas a padecer una enfermedad contagiosa, no se les permitirá permanecer en la casa.
• Familias registrados que están en un cuarto por mas que 30 dias, están requeridos a cambiar cuartose n RMHC cuando estan disponible, por razones de manteniendo.

He leído, entiendo y estoy de acuerdo ·en cumplir las guias de la Casa Ronald McDonald. Entiendo que violaciónes o negligencia de las guías, según lo determinado por el personal de RMHC a su sola discreción, puede resultar en la pérdida inmediata de acceso a la casa . También eAtendemos que si una familia con mayores necesidades necessita un cuarto, nos puede pedir que encontremos alojamiento alternativo y en este caso, se nos dará un periodo de gracia de 24 horas para desocupar el cuarto.
